What is Charm PHR?
Charm PHR is short for Charm Patient Health Records (PHR). It is a secure HIPAA compliant online health records system provided by CharmHealth.

How do I log in?
The first time you log in to Charm you will need to do so by following the instructions in an email invitation from Dr. Irina Strelyuk, NMD, PLLC with the subject line “Patient Portal Registration”. If you are not seeing this email in your inbox, please check the spam or junk folder.
NOTE: if you have a Charm account set up with any other clinic or provider you will need to log out of that account before clicking on the link in the invitation email and following the steps to verify your date of birth and enter your existing login ID and password.
How do I upload labs, imaging and other documents to share with my provider?
In your Charm patient portal, go to the Documents tab/cube. Upload the file (must be in PDF format) and select the practitioner you would like to share the document with (if you do not do this step, the document will be uploaded and only visible to you).
All uploaded labs, imaging, and any other medical reports MUST include your name and date of birth. Do NOT upload screenshots of selects lab markers. All documents must be uploaded in a PDF format.
